In addition, please provide any addendums including contract assignments relating to your contract.

  • Fax a copy of the following to 425-822-5305 (required prior to first loan*):
    • "Assumed Name (DBA) Certificate" filed with the county(ies) or Secretary of State

CORPORATION ONLY (S OR C CORPS ) (NOT FOR LLC’S OR LIMITED PARTNERSHIPS)

  • Fax copies of each of the following to 425-822-5305 (required prior to first loan*):
    • Copy of Social Security card of Signer(s) and Guarantor(s)
    • Copy of Driver's License of Signer(s) and Guarantor(s)
    • IRS Form W-9 forporation
    • Two current months bank statements of Corporation
    • Two current months bank statements of Signer(s) and Guarantor(s)
    • Certificate of Existence filed with the SOS (Need current "Active Status")
    • Certificate of Formation (Articles of Incorporation) filed with SOS
    • Corporate Bylaws and any amendments made to the original bylaws
    • "Certificate of Good Standing" from SOS (some states just designate with "Active Status") from organization State (and from State where property is located if different)
    • "DBA" filed with SOS (or with County in some State) if Corporation to go by assumed name

LIMITED LIABILITY COMPANY (LLC) ONLY (NOT FOR CORPORATIONS OR LIMITED PARTNERSHIPS)

  • Fax copies of each of the following to 425-822-5305 (required prior to first loan*):
    • Copy of Social Security card of LLC Signer(s) and Guarantor(s)
    • Copy of Driver's License of Signer(s) and Guarantor(s)
